Sometimes, the surge level is too low to notice, or certain health conditions make tracking difficult. It often takes a couple of cycles to test the LH surge properly, so you may miss it even when you ovulate.


Role of LH Surge In Normal Ovulation

As your body begins the ovulation process, the oestrogen level rises due to the presence of mature eggs ready to release. The high oestrogen levels signal your brain to release the follicle-stimulating hormone (FSH) and LH. Around days 6 to 10 of your cycle, the egg matures because of FSH, leading to its maturity after day 10.

When your body experiences the LH surge, this egg is released typically around day 14 of your cycle. You can expect ovulation anywhere between 12 and 24 hours after peak LH. The chances of conception are very high in this period.


Can Ovulation Happen Without an LH Surge?

In another case, you may have lower baseline levels of LH surge, which still show an ovulation. Sometimes, stress, poor diet or pituitary disorder can result in a lower or undetectable LH surge.


Medical Conditions Affecting LH Surge

Certain medical conditions can affect your LH surge levels, leading to no ovulation.

1. PCOS (Polycystic Ovary Syndrome): If you have PCOS, your body may produce higher levels of androgen hormones, which can lead to hormonal imbalances. These changes might affect how your eggs mature and, in some cases, may prevent ovulation from happening due to the absence of the necessary LH surge.

2. Hypothalamic amenorrhea: If you suffer from hypothalamic amenorrhea, you may get irregular or no periods at all. The hypothalamus is responsible for all your regular bodily functions. However, when it has a problem, it may secrete low levels of gonadotropin-releasing hormone (Gnrh). The Gnrh triggers the release of FSH, oestrogen and LH for the regular menstruation cycle.

3. Premature Ovarian Failure: If you’re experiencing POI, it means your ovaries may start to function less effectively before the age of 40. This change can affect the balance of several hormones in your body, including LH, which may lead to disruptions in the ovulation process.

Experiencing reduced fertility is common after the age of 40 years because of menopause. However, irregular periods starting at a young age are a common sign of POI. It may even start as early as your teenage years.


False Negatives in LH Testing

If you rely on OPKS for testing the LH surge, you may get negative results even when you ovulate. This might happen if the test isn’t used correctly or taken after ovulation has already occurred.

It's best to take your LH test in the early afternoon to improve accuracy, as LH levels are often most detectable around this time. You can also take another test in the evening if needed. Properly timing your LH testing can help you avoid false negatives and track your ovulation window better.


Signs You Are Ovulating Without Detecting an LH Surge


You can track other body signs of ovulation. These include:

  • Cervical mucus consistency: The cervical mucus becomes thick and cloudy, almost resembling the egg-white consistency during ovulation. It is usually thinner and clearer before ovulation.
  • Basal body temperatures (BBT): The body temperature rises slightly during ovulation. The best time to check your basal body temperature is right after you wake up, before you get out of bed or do any activity. A slight rise in temperature typically occurs after ovulation, so your fertile window usually begins two to three days before this increase.
For accuracy, you can also get ultrasound scans to see the presence of a mature egg. This will increase your chances of conception.

Having one or two irregular cycles is common because of your lifestyle. However, if you have a persistent irregular cycle or medical conditions like PCOS, you should consult your doctor.
