Can Varicocele Affect Sperm DNA Even When Sperm Count Is Normal?

A normal sperm count does not always mean sperm DNA is healthy. Varicocele may be linked to sperm DNA damage in some men, potentially affecting fertility even when routine semen results appear normal. Here, our expert explains what men should know about testing and treatment.

Pregatips
A semen analysis provides useful information about male fertility, but it does not reveal every aspect of sperm health. Some men may have normal test results yet still experience difficulty conceiving or repeated pregnancy loss. Sperm DNA integrity is another factor doctors may consider during a fertility evaluation.
Varicocele may be associated with changes in sperm DNA, even when routine semen results appear normal. Understanding how these factors connect can help men interpret their fertility results and plan next steps in their care.

To understand how varicocele can affect sperm DNA quality even when sperm count is normal, and its possible impact on male fertility, we spoke with Dr Rashmi Niphadkar, Fertility Specialist at Nova IVF Fertility, Baner, Pune.

What Is a Varicocele?

A varicocele is an enlargement of the veins that drain blood from the testicle. It is similar to varicose veins in the legs.

A varicocele may affect the testicle by changing the local environment around it. In some men, this can be associated with:

  • Increased oxidative stress
  • Higher testicular temperature
  • Changes in blood flow
  • Reduced sperm production or quality

Can Varicocele Affect Sperm DNA?

Yes, it can. Varicocele has been associated with increased sperm DNA fragmentation in some men. One possible explanation is oxidative stress, which can damage the DNA inside sperm cells.

This means that a man may have a reasonable sperm count but still have a problem with sperm DNA integrity.

However, having a varicocele does not automatically mean that sperm DNA is damaged. The effect can vary depending on factors such as the varicocele's size and severity and the individual's overall reproductive health.

Not every man with varicocele experiences sperm DNA damage or fertility problems. A semen analysis and, in selected cases, sperm DNA fragmentation testing may help assess sperm health and guide further treatment based on the couple’s fertility history.Dr Rashmi Niphadkar, Fertility Specialist at Nova IVF Fertility, Baner, Pune

Can Sperm DNA Be Damaged Even With a Normal Semen Analysis?

Yes. A routine semen analysis mainly evaluates characteristics such as:

  • Sperm count
  • Sperm concentration
  • Sperm movement
  • Sperm shape
  • Semen volume
It does not directly measure whether the DNA inside the sperm is intact.

This is why a normal semen analysis does not always rule out every possible male fertility problem. Recent research has also highlighted that men with semen parameters within reference ranges can still have reproductive problems.

Does Every Man With Varicocele Need a Sperm DNA Test?

No. Sperm DNA fragmentation testing is not recommended as a routine first-line test for every man being evaluated for infertility.

A fertility specialist may consider additional testing depending on:

  • Duration of infertility
  • Varicocele findings
  • Previous fertility treatment
  • Recurrent pregnancy loss
  • Previous pregnancy or IVF failures
  • Semen analysis results
  • Other male fertility factors
Base the decision on the individual's clinical history rather than the presence of a varicocele alone.

When Should Men With Varicocele Consider a Fertility Evaluation?

A fertility evaluation may be appropriate when:

  • Pregnancy has not occurred despite regular unprotected intercourse
  • A varicocele has been diagnosed
  • Semen analysis is abnormal
  • There is a history of fertility problems
  • The couple has experienced recurrent pregnancy loss
  • There have been repeated unsuccessful fertility treatments
  • There is testicular pain, shrinkage, or another testicular concern

Can Treating Varicocele Improve Sperm Health?

Treatment depends on the individual situation. Not every varicocele requires treatment.

When treatment is appropriate, varicocele repair may improve some semen parameters in selected men. Research has also reported improvements in sperm DNA fragmentation after varicocele repair, but the evidence is not strong enough to suggest surgery for every man with a varicocele.

Doctors generally consider factors such as:

  • Whether the varicocele can be felt on examination
  • Fertility status
  • Semen analysis results
  • Testicular size or function
  • Pain or other symptoms
  • The couple's reproductive plans

What Can Men Do to Support Sperm DNA Health?

Men can also focus on lifestyle habits that support overall sperm health:

  • Avoid smoking
  • Limit alcohol
  • Maintain a healthy weight
  • Exercise regularly
  • Get adequate sleep
  • Eat a balanced diet
  • Avoid unnecessary heat around the testicles
  • Manage medical conditions
  • Seek treatment for infections when needed
Varicocele may affect sperm DNA quality in some men, even when sperm count is normal. However, its impact varies. Medical evaluation can help identify relevant fertility concerns and guide decisions about further testing or treatment based on individual needs.

FAQs on Can Varicocele Affect Sperm DNA Even When Sperm Count Is Normal?

  1. Does varicocele always cause sperm DNA damage?
No. Some men with varicocele may have increased sperm DNA fragmentation, while others may have normal sperm DNA.
  2. Can sperm DNA damage affect pregnancy?
Higher sperm DNA fragmentation has been associated with poorer reproductive outcomes in some studies, including pregnancy loss, although the clinical significance varies between individuals.
  3. Is sperm DNA fragmentation testing done with a normal semen analysis?
It can be considered in selected situations, but it is not routinely recommended as part of the initial evaluation of every infertile man.
