What Is Ovarian Reserve Discordance?

Ovarian reserve refers to the number of eggs remaining in the ovaries. Ovarian reserve discordance means that ovarian reserve tests provide results that do not appear to agree

For example, a woman may have:

  • A lower AMH level but a relatively high AFC
  • A higher AMH level but fewer antral follicles seen on ultrasound

AMH and AFC: How Are They Different?

AMH is a hormone produced by small follicles in your ovaries. A blood test can measure the amount of AMH circulating in your body. It is relatively convenient because it usually does not need to be timed to a specific day of the menstrual cycle.

AFC, on the other hand, involves a transvaginal ultrasound. Your doctor counts the small antral follicles visible in both ovaries, usually during the early part of your menstrual cycle.


Why Can AMH and AFC Not Match?

Several factors can cause AMH and AFC results to differ:

  • Natural variation: Both tests are estimates, so they may not provide the exact same picture of ovarian reserve.
  • Ultrasound differences: AFC can vary depending on how clearly follicles are seen, the equipment used and the clinician performing the scan.
  • AMH changes: AMH is relatively stable but can vary. Different testing methods may also affect the result.
  • Hormonal contraception: Birth-control hormones can temporarily influence AMH or AFC.
  • Polycystic ovaries: A higher number of small follicles and elevated AMH can sometimes occur with polycystic ovary morphology.
  • Age and individual biology: Ovarian reserve changes with age, but the pattern varies for each woman.

Does Discordance Mean Something Is Wrong?

A mismatch between AMH and AFC does not necessarily mean poor fertility or an ovarian problem. Doctors consider your age, menstrual history, fertility goals and other test results. If the difference is significant, they may review your medications, test timing and ultrasound findings before recommending further evaluation.

What Does It Mean for IVF?

AMH and AFC are particularly useful for predicting how the ovaries may respond to fertility medicines used during IVF.

For example:

  • A lower ovarian reserve marker may suggest that fewer eggs could be retrieved after stimulation.
  • A higher result may indicate a stronger response, although it can also be associated with a greater risk of ovarian hyperstimulation in some patients.
AMH and AFC can help plan IVF, but they cannot predict its outcome. Age, egg and sperm quality, embryo development and overall reproductive health also play a role.

Can You Still Get Pregnant With Low AMH or AFC?

Yes. A low AMH or low AFC does not mean pregnancy is impossible.

These tests primarily indicate ovarian reserve and expected response to stimulation. They do not directly measure whether ovulation is occurring or whether an egg can be fertilised and develop into a healthy embryo.

If you are trying to conceive, your doctor may consider your age and how long you have been trying alongside other investigations rather than relying on ovarian reserve testing alone.

When Should You Speak to a Fertility Specialist?

Consider discussing discordant AMH and AFC results with a fertility specialist if:

  • Your results are very different from each other
  • You are planning IVF or egg freezing
  • You have concerns about declining fertility
  • You have had previous ovarian surgery or fertility treatment
  • Your results appear unexpectedly low or high for your age
  • You want to understand your reproductive options
