Vulvar Psoriasis: Symptoms, Treatment, and Pregnancy Planning

Vulvar psoriasis can cause itching, burning, soreness and changes in the skin around the vulva. These symptoms can sometimes look like a vaginal infection or another skin condition. Our experts explain the symptoms, causes, and how your treatment may vary if you are planning a pregnancy.

Pregatips
Psoriasis is a chronic inflammatory skin condition. It can affect different parts of your body, including the vulva. It may cause redness, itching, burning and soreness. This skin condition is not contagious. It also doesn't spread through sexual contact. It develops due to our body’s internal inflammatory responses to certain triggers.
However, the symptoms may affect your comfort levels, sexual intimacy, and quality of life. We spoke to Dr Sarada M, Senior Consultant Obstetrician & Gynaecologist at Yashoda Hospitals, Hyderabad, about symptoms, treatment, and pregnancy planning with vulvar psoriasis. She emphasised that if vulvar symptoms persist for longer, a medical evaluation is required, especially if you are planning a pregnancy.

What Are the Symptoms of Vulvar Psoriasis?

The vulva is the external skin surrounding your genitals. This condition may lead to the following symptoms:

  • Itching
  • Burning sensation
  • Soreness
  • Redness or darkness of skin
  • Skin dryness or cracking due to irritation
  • Discomfort during sexual activity
  • Flare-ups with triggers like friction, sweating or tight clothing
Psoriasis may also affect the other parts of your body, including

  • Scalp
  • Elbows
  • Knees
  • Other skin folds
Your dermatologist can help you distinguish psoriasis from other conditions by physically examining the affected area.

Psoriasis Vs Other Vulvar Conditions With Similar Symptoms

Vulvar psoriasis can resemble several conditions. The diagnosis may sometimes be delayed, as certain infections and other skin conditions can cause similar changes.

The differences below may help you understand why an examination is important.

Other Vulvar ConditionsWhat differentiates it from vulvar psoriasis
Vaginal thrushIt is usually associated with thick, white vaginal discharge
Contact dermatitisSymptoms generally flare up upon exposure to triggers
Lichen sclerosusThe skin is visibly pale. The skin becomes thin and may have small cracks
Lichen planusIt is usually associated with raw areas or erosions around the vaginal opening or inside the vagina
However, self-diagnosis and self-treatment may prove harmful. Therefore, consult a dermatologist or gynaecologist for an accurate diagnosis and appropriate treatment.

How Is Vulvar Psoriasis Treated?

Treatment usually depends on the severity of symptoms and the site affected.

Treatment optionHow it helps
EmollientsFragrance-free moisturisers reduce dryness, friction and irritation.
Topical corticosteroidsControl inflammation. Potent preparations require medical supervision.
Other topical treatmentsA dermatologist may recommend calcineurin inhibitors when appropriate.
Dr Sarada advises the following tips to prevent exposure to triggers:

  • Avoid scented products
  • Do not use harsh cleansers
  • Do not wear tight clothing
  • Choose loose-fitting underwear
  • Avoid unnecessary rubbing or scratching

What About Vulvar Psoriasis When Planning Pregnancy?

Speak to your dermatologist and gynaecologist before conception to review your medicines. Some psoriasis treatments may be unsuitable during pregnancy or require a planned washout period.

  • Emollients: Generally suitable during pregnancy.
  • Topical corticosteroids: May be used when needed under medical guidance, with the preparation and duration chosen carefully.
  • Narrowband UVB phototherapy: May be considered when additional treatment is needed.
  • Acitretin: Must not be used during pregnancy. Pregnancy should be avoided for three years after stopping it.
  • Methotrexate and tazarotene: Require specific pregnancy precautions and medical advice before conception.
Dr Sarada stresses that you should never stop or change prescribed treatment without consulting your doctor. Your dermatologist and obstetrician can help plan suitable treatment before conception and during pregnancy and breastfeeding.

Psoriasis may improve or worsen during pregnancy, and flare-ups can occur after delivery. Regular follow-up helps maintain symptom control while allowing your treatment plan to be reviewed as your needs change.
