- Reduced egg quality, especially with increasing maternal age
- Poor sperm quality, including sperm DNA damage
- Chromosomal abnormalities in the embryo
- Oxidative stress, which can damage egg and sperm cells
- Natural cell division errors during early embryo development

How Do Embryologists Grade Embryo Fragmentation
Embryologists do not assess embryo fragmentation in isolation. They evaluate it alongside the embryo's stage of development, cell number, cell symmetry, and other morphological characteristics to determine the overall embryo grade.According to the ESHRE/ALPHA Consensus on Oocyte and Embryo Static and Dynamic Morphological Assessment, embryo fragmentation is generally classified as:
- Less than 10% fragmentation: Minimal fragmentation
- 10–25% fragmentation: Mild fragmentation
- More than 25% fragmentation: Severe fragmentation
