What is Cervical Insufficiency?

Cervical insufficiency is a condition in which the cervix (the lower, narrow part of the uterus) shortens or softens and begins to open early in pregnancy without significant pain or noticeable contractions. Normally, the cervix stays closed during pregnancy and opens at the beginning of labour. If the cervix opens early, it may not hold the pregnancy in the normal way, followed by some risks such as:

  • Miscarriage in the second trimester
  • Premature birth
  • Cervix opening too early
  • Premature delivery
Nonetheless, each pregnancy is unique, and being diagnosed with cervical insufficiency does not necessarily mean that the pregnancy will end up with a miscarriage or a premature birth.

What Causes Cervical Insufficiency?

In most cases, the actual cause may not always be identified. Dr Shobha Gupta highlights a few factors that can increase the risk of cervical insufficiency. Some of these may include the following:

  • A previous second-trimester loss
  • A previous spontaneous premature delivery
  • Previous procedures on the cervix
  • Damage to the cervix

Cervical changes can sometimes happen without presenting any visible symptoms. Thus, follow-up is essential for women who are at risk of cervical alterations. Doctors typically recommend a transvaginal ultrasound scan to get a clearer look at the cervix and identify any changes or abnormalities.

What Treatment May Be Recommended?

Treatment for cervical insufficiency during pregnancy depends on the patient. Dr Shobita Gupta explains that the treatment plan uses criteria such as history of pregnancy, findings at the cervix, and the gestational age.

Treatment options include the following:

  • Cervical Cerclage
In a cervical cerclage procedure, also known as the cervical stitch technique, stitches are made around the cervical site to prevent early opening. The procedure is performed after the evaluation of medical history and cervical findings.

  • Vaginal Progesterone
For certain pregnancies, vaginal progesterone is initiated in cases of a short cervix. The hormone is placed inside the vagina during pregnancy. It helps maintain cervical stability and lower the risk of premature birth. It is advised based on individual pregnancy factors and cervical measurements.

  • Cervical Pessary
Here, a silicone device may be placed around the cervix, although its use depends on individual circumstances and local clinical practice.

  • Regular Monitoring
Sometimes, pregnant women may be monitored through transvaginal scans by evaluating the cervical length to guide further treatment.

Cervical insufficiency requires close obstetric supervision, but women should not assume that the diagnosis automatically means they will lose the pregnancy. The management plan depends on their previous pregnancy history, cervical findings, and gestational age.”

Which Warning Signs Should You Watch For?

  • Bleeding from the vagina
  • Fluid discharge
  • Any abnormal discharge from the vagina
